The aftershocks of cancer affect one's emotional, physical, cognitive, and social lives.

Before and during treatment, there are frequent follow-up appointments for monitoring one’s response to therapy. However, after completing cancer therapy, many survivors are left with the question, now what? It can be a scary time of transition in a cancer survivor’s life. With advanced treatment options, many patients are living longer after cancer.  So what happens next?

The Cancer Survivorship program combines evidence-based medicine with integrative and wellness services for patients and caregivers affected by any type of cancer. Our team includes a full spectrum of clinicians and wellness providers to help meet a multitude of needs.

If you or your loved one have worries about the impact of cancer and its therapy, consider a visit with our cancer survivorship team. You will receive a comprehensive Survivorship Care Plan that is specific to you and your cancer experience, serving as your guide on your survivorship journey of health and wellness. We also offer a range of community and online resources and programming that may be of interest.

Your input is a crucial step as we develop goals for your wellness.

Your first visit will include

  • Completion of a questionnaire to help us to better understand your health and wellness needs
  • A one hour appointment that allows for plenty of time for discussion
  • Discussion and explanation of your personalized Survivorship Care Plan
  • Referrals as needed to genetics, performance therapy, nutrition, social work, endocrine bone health, and psychology

What is a Survivorship Care Plan?

It is a detailed record of your cancer experience. It is important to keep on file so that other health care professionals that you see in the future will have the necessary information on how best to work with you to monitor your health.  It can also be updated when your medical condition changes.

Survivors Empowered!

You and one of our specialized Nurse Practitioners will review your personalized Survivorship Care Plan together, which has already been discussed with your cancer treatment team. A paper and electronic copy will be provided to you, your treatment team, and primary care provider.
